FEES & INSURANCE

Our fees are based upon the type and extent of treatment you require. We request that you pay your fee at the time of your office visits. At no time, however, do we want financial hardship to come between you and appropriate care. If you find yourself in financial difficulties, please call our business office at 610-439-8551, prompt #2, to arrange a comfortable method and amount of payment.

We are a participating office with Pennsylvania Blue Corss/Blue Shield and receive payment directly from them where applicable. We will be happy to file the claim for you, but you will be responsible deductibles and non-covered services. For instance, most Blue Shield plans do not cover office visits.

MEDICARE

We participate with Medicare and will file the claim for you. Since Medicare pays only 80% of the approved amount, you will be billed for the remaining 20%, as well as for your yearly deductable amount.

MAJOR MEDICAL AND COMMERCIAL INSURANCES

We will supply you with an itemized statement at the time of your office visit. This statement will contain all the information you need to submit your claim to the insurance company.

HMO & MANAGED CARE

If you belong to an HMO or Managed Care program, please obtain a referral from your primary care physician. Kindly bring this referral form at the time of each visit as we cannot treat patients without the necessary referral forms. Please bring your insurance card with you to your appointment.
